Please complete the questionnaire if you are aged 50 years and over and have used our services or may use our services in the future. You can also complete it as a carer, spouse, partner or relative of someone over 50. Your views are very important in helping us find out how we are doing as a service and how our services can be improved.

The Welsh Ambulance Services NHS Trust provides a number of services across Wales:

• Unplanned care such as 999 emergency response
• Pre-planned care for example the Patient Care Service that provides patient transport to and from hospitals, clinics and day centres
• Volunteer Ambulance Car Service, where dedicated volunteers take patients with limited mobility to and from hospital appointments
• NHS Direct Wales is also part of the Welsh Ambulance Services NHS Trust and is a telephone based health information and advice service which is available 24 hours a day
